Clearly defined conceptual sprint plan laying out deliverable and a road-map that identifies dependencies between work-streams is critical to track transformation programs. The Agile methodology should ensure the life cycle of story is clearly followed from definition to approval by Product owner and Business team.

Is Product owner fully responsible for product functionality?

Product owner has the single responsibility to ensure that the functionality delivered meets business expectations. Typically the Product owner is supported by a team that are directly involved in the project as SMEs and a broader team is available to him to provide guidance on complex issues requiring broader consensus.

Should requirements, development and testing be completed in a single sprint?

It is beneficial to split the requirements to be done a sprint ahead of development and testing. This gives time to elaborate the requirements and development team can define a solution that addresses all the nuances of the requirements.
